General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.
For an already recognised affection de longue durée (ALD), the care protocol is the medical document describing care requested in relation to the condition. It may be created electronically or on paper.
Your treating doctor prepares it in consultation with you and the other doctors involved in your condition. The Assurance Maladie medical service then examines it. Ask which sections concern your current follow-up.
Ask the doctor to explain the condition and the care, tests or professionals recorded. Some ALDs use a simplified declaration, so the level of detail in the protocol can vary.
Keep the copy provided by the doctor. If the form was completed electronically and with your agreement, the patient section can be viewed in Mon espace santé. Ask the doctor which copy to retain.
Find the end date of ALD coverage at the bottom right of the protocol and discuss the next steps with the doctor before then. Renewal or another follow-up arrangement depends on your health; the date alone does not determine that care should stop.
The full protocol is personal and confidential: show it only to doctors treating you or Assurance Maladie medical advisers. It does not mean that all care is covered under the same conditions.
