Lost vaccination record: rebuilding the documented history

Find available records, distinguish documents from memories, then review the information with a healthcare professional.

General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.

Losing a vaccination record leaves questions about what you received. Start by rebuilding the available information without filling gaps from assumptions: the aim is to make any uncertainty clear to the healthcare professional.

Check your Mon espace santé file and documents kept at home. Vaccination information may be there, but that file alone does not prove that every older vaccination has been entered or can be recovered.

For each record you find, keep a copy and note the vaccine recorded, its date and where the document came from. Put memories without documentation in a separate list, alongside questions needing verification.

Ask professionals or organisations that previously cared for you what information remains available. A focused request with an approximate period and place can help locate records without assuming that they will be found.

Take this material to a doctor, pharmacist, nurse or midwife. They can review it in light of your age and health. Ask what is established, what remains uncertain and how to keep the revised record.

Your next step should follow that professional review. For other prevention questions, you can also visit our Mon bilan prévention page if you would like to prepare a broader discussion about your health.

Flu and Covid-19: the 2026–2027 campaign

The national flu and Covid-19 vaccination campaign starts on 13 October 2026. In Doctolib, choose “Vaccination antigrippale (Grippe)” or “Vaccination COVID-19” for the vaccination you want to discuss.

Bring your carte Vitale and, for flu, your coverage voucher if you have one. The professional checks the recommendations and coverage conditions that apply to you.

The pharmacy checks whether vaccination and coverage apply based on the vaccine, your age and your circumstances.
