If your physical Carte Vitale is lost or stolen, notify your health insurance fund immediately. For CPAM, you can declare this in the ameli account’s Démarches section or through the fund.
Before confirming, check the reason and the insured person concerned. The declaration makes the old card unusable, even if you find it later.
After declaring the loss in ameli, request the new card in Démarches. Prepare a digital identity photo and a valid identity document, then check and confirm the order.
Without an ameli account, you can still declare by telephone, post or at a CPAM reception office. The fund sends a form; if it contains an error, contact the fund rather than correcting it yourself.
While waiting, obtain proof of health insurance entitlement to present to the professionals you consult. Request it in the ameli account’s Documents section or directly from the fund.
Track the order under Démarches, then Vos cartes, and check the information received with the new card. If you find the old card after replacement, return it to the fund; do not use it again.
