General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.
To check information in your French Dossier Pharmaceutique, or DP, request access to your data from a pharmacist permitted to consult it. You can also exercise this right through the French National Order of Pharmacists, known as CNOP. Ask which way of providing the information suits your request.
In the copy, you can check information about medicines and vaccines dispensed to you with or without a prescription: their characteristics, quantities, dates and details of dispensing and prescribing. These are dispensing records; on their own, they do not prove which doses you actually took.
If you also want records of actions carried out on the DP, specify this. Through CNOP, the right of access covers the DP data and all these action records. Through a permitted professional, it covers the DP data and records of actions carried out in the pharmacy or establishment where that professional works.
For a request to CNOP using the content-access form, Service Public describes a form to print, complete by hand, date and sign, then send by email or post with the requested documents. Prepare a copy of both sides of your identity document and, where applicable, documents for the legal representative or guardian. This process provides a paper copy by registered post.
If you identify a dispensing error in the copy, ask the pharmacist to check it. The French National Order of Pharmacists’ page says you can exercise your right to correct a dispensing error through any pharmacist. Identify the entry concerned and explain the discrepancy to prepare this check.
When preparing the discussion, distinguish your request for a copy, your request for action records and your report of a dispensing error. Note the entries you have questions about and show useful documents to the pharmacist. The copy helps check dispensing data; it does not replace the prescription or authorise changing treatment yourself.
