General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.
A therapeutic patient education programme proposed for a chronic condition helps you develop or maintain skills for living with it. It combines information, learning and support as part of care.
The first discussion identifies your needs, expectations, personal goals and resources. This educational needs assessment helps the team build a learning programme with you.
Prepare examples of everyday difficulties, what you already understand and what you would like to learn. Mention difficulties with understanding, language or a disability so the discussion can be adapted.
Discuss the skills to prioritise in relation to your goals and care. Ask how individual or group sessions will help you work on these objectives with the team.
Explain whether you would like relatives involved, and whether they would also like to participate. Ask how professionals will coordinate educational information and what role you can take in that coordination.
Ask how the skills you have developed and your remaining needs will be reviewed. Educational follow-up can be adapted as your health, care or life changes; it forms part of your medical care.
