A paper care claim form lets you request reimbursement from your CPAM. Before submitting it, complete your section and sign in the space provided for the insured person.
If the care concerns a dependent, distinguish their details from those of the insured person. The instructions on the back explain the fields; the insured person signs.
The Compte ameli app accepts forms from a doctor outside a care facility, for an amount no higher than 100 euros. The limit is five claims in thirty days, including dependents.
In the Démarches section, photograph the form and enter the requested details: care recipient, doctor identification and fees. Check the photo quality, then confirm the submission.
After successful transmission through the app, do not also post that form to your CPAM. Keep the original for twenty-seven months: digital submission is no reason to discard it.
If this route is unsuitable or submission fails, send the signed form by prepaid post or leave it at an authorised reception point. Check your fund’s address and estimated processing time.
