General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.

For a planned admission to a hospital or clinic, formalities are handled by the admissions department. Before your stay, ask where to report and which documents to prepare.

Gather an identity document, your updated Carte Vitale, proof of health insurance entitlement and your supplementary insurance card or certificate if you have one.

Hospitalisation related to a work accident or occupational disease also requires the corresponding form. If supporting evidence is missing, contact admissions to clarify which documents fit your situation.

Your médecin traitant’s letter, examination results and health record booklet may be useful. Ask the department which documents are expected for your file.

After registration, ask for the hospitalisation or situation certificate. To serve as a sick leave notice, it must show admission and discharge dates, identify the establishment and bear a stamp and signature; outpatient consultations are excluded.

Send this certificate within forty-eight hours to your fund and, as applicable, your employer or France Travail. The establishment handles the formalities if your health prevents you; your employer need not know the medical reasons.
