A barking cough in a child: recognise emergencies and seek same-day assessment

A hoarse voice and noisy breathing need examination. Trouble swallowing, drooling and severe breathing difficulty take priority.

For suspected croup, call 15 or 112 promptly if the child is under six months old, has a heart or chronic condition, refuses to drink or stops feeding. This urgent pathway also applies if they are very pale, turn blue, breathe irregularly, have severe breathing difficulty, agitation, drowsiness, confusion or marked fatigue. If they struggle to swallow and drool with a high fever, refuse to swallow or remain sitting, do not lay them down.

Croup can cause a hoarse voice, a harsh barking cough and an abnormal sound when breathing in. It often follows a cold and may appear at night. Inflammation narrows the airway at the child’s larynx.

When none of the urgent situations above applies, suspected croup needs same-day medical examination even if the child seems generally well. Ameli states that it should not be managed through self-medication. Contact the doctor or paediatrician to assess breathing.

Reassure the child and offer calm activities while waiting for appropriate advice. Agitation can make breathing difficulty worse. Avoid tobacco smoke and do not give cough medicine without medical advice.

If the child has already been examined but the cough worsens or fever develops, contact the doctor again that day. Also seek same-day reassessment for cough or breathing difficulty persisting beyond two to three days. Marked worsening or new warning signs requires the emergency pathway.

When calling, describe the age, onset, breathing sound and ability to drink or swallow. These details help guide examination; they cannot establish that croup is harmless. General prevention discussions come after the necessary care.
