Weekly methotrexate: prevent an accidental daily schedule

The prescribed day must be clear for a weekly oral treatment. Taking it more often than prescribed requires immediate medical contact.

General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.

If oral methotrexate prescribed once a week has been taken daily or in a greater amount than prescribed, contact a doctor or the nearest hospital emergency department immediately. The leaflet says to take the pack with you. Do not wait for a prevention discussion to have the error assessed.

The cited IMETH leaflets specify once-weekly use for rheumatoid arthritis, psoriatic arthritis and psoriasis. This applies to these weekly oral treatments. It does not establish the schedule for every methotrexate use, including a cancer treatment protocol.

The product information requires the prescriber to specify the weekday on the prescription. The leaflet says to use the same day each week. Both the weekly frequency and the day must be identifiable; the number of tablets alone does not explain the schedule.

The prescriber must ensure that the patient or caregiver can follow the weekly regimen. If a relative helps prepare medicines, have the professional clarify the same day and frequency for everyone involved. A new helper must not turn this schedule into a daily routine.

The leaflet warns that daily use instead of weekly use can be fatal. This is a potentially serious frequency error, not just an organisational choice. Tell the doctor what was actually taken and when, without calculating a treatment correction yourself.

If the day or frequency is unclear on the documents, show the prescription and pack to the doctor or pharmacist for explicit instructions. Do not independently spread a weekly prescription across daily doses or apply another medicine’s missed-dose rule.
