Thyroid nodule with normal TSH: what still needs checking?

TSH reflects thyroid function. Ultrasound helps decide whether a nodule needs fine-needle sampling or monitoring.

General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.

A nodule may be found by feeling the neck or during an examination for another reason. Even without symptoms, discuss the finding with your doctor to decide which assessment is useful.

Normal TSH indicates normal thyroid function. That result alone does not describe the nature of the nodule: blood testing and assessment of the nodule answer different questions.

Ultrasound describes features including the nodule’s size, contents and borders, and examines nearby lymph nodes. The EU-TIRADS score summarises characteristics that help the doctor assess risk.

Depending on the findings, fine-needle sampling may be proposed to analyse cells taken with a thin needle. This is not automatic for every nodule; the doctor may choose monitoring alone.

For a benign nodule that does not secrete hormones, follow-up may combine clinical examination and ultrasound. The doctor sets the intervals, which may lengthen if it remains stable; a change may lead to repeat sampling.

Bring your TSH result, previous ultrasound reports and any fine-needle sampling results. Ask what supports the choice between sampling and monitoring, who will explain the result and when the next check is planned.
