Pharmacien correspondant: preparing the declaration and prescription

Designation requires the pharmacist’s agreement and coordination with your usual doctor. Permissions recorded on the prescription remain separate steps.

General information, not an individual diagnosis. Ask a healthcare professional if you are unsure about your situation.

The patient designates a pharmacien correspondant with Assurance Maladie, with the pharmacist’s agreement. The pharmacist must belong to the same coordinated-care organisation as the patient’s usual doctor.

Before preparing the declaration, ask the professionals whether they share that organisation. Your usual pharmacy does not automatically become your designated pharmacien correspondant.

The patient and pharmacist complete the declaration form together. The patient sends it to their health insurance fund and the pharmacist keeps a copy; the pharmacist informs the usual doctor.

Also prepare the usual doctor’s prescription. It must expressly authorise renewal by the pharmacien correspondant: ask which treatments are covered and for how long.

Any dose adjustment requires a separate permission on the prescription. Designation alone therefore does not authorise every intervention; ask the professionals to explain the entries.

The prescription and all renewals together cannot exceed twelve months, with specific limits for some medicines. Ask the doctor to clarify the end date and subsequent follow-up.
